To contact the Middletown High School Health Office please call 326-1590. If your child will be absent form school, please call the attendance office at 326-1568 or 326-1567.

At the beginning of the school year emergency information cards are sent home. These cards provide the school with vital information in the case of an emergency. It is important that the cards be returned as soon as possible, and that the information be correct and current.
